Mach Martial Arts Masters will fill out Independent Physical Education (PE) Waiver Forms – only the coach/instructor portion.
All other portions must be pre-filled out by you before submitting the request to the Master. 

Check your school website for deadlines, permission forms and instructions on this process. If you have questions and/or concerns, please inquire with your school Physical Education (PE) Department.
