Membership Extension Request Form
Please DO NOT submit a Membership Extension Request if you are making up classes for the same period.
If your membership extension is longer than 1 month, your membership rate will revert to the current market rate when you resume classes. You can view current market rate for membership here: https://machtkd.com/classes/pleasanton/
You will automatically receive a copy of the completed form via email.
Example scenario for an extension request: If student’s membership is billed on the 2nd of every month and will miss class from 09/15/21-09/25/21 (10 days), your membership will be extended from the current billing cycle date of 10/02/21 + 10 days = 10/12/21. The new billing cycle date is 10/12/21 and you will now be charged on the 12th of each month. You will then resume classes on 09/26/21 (the day after your requested 10 day extension) even though you will be billed on 10/12/21.
